It’s time to make plans and sign up for this year’s 4th of July event by completing the Park Vendor Application form below. We are looking forward to a wonderful park event and look forward to you joining the festivities!

In consideration of accepting this, executors and administrators, waive and release any and all rights and claims I/we may have against the Middleton Chamber of Commerce, the City of Middleton, and all other parties and representatives, successors and assigned for any loss or damage to property or persons. I agree to all fees, rules, and regulations as state in the application and guidelines.
