It’s time to make plans and sign up for this year’s 4th of July event by completing the Park Vendor Application form below. We are looking forward to a wonderful park event and look forward to you joining the festivities!



Prior to applying to be a vendor review the 2023 vendor guidelines.







All food venders must also review the 2023 mobile food vendor guidelines.

I understand that access to electricity and water are not available
I understand that all vendors must fully comply with all regulations for sales tax, mobil food unit guidelines, Health Department guidelines, etc. and will have all proper paperwork and permits on site

I/My organization and/or business intends to have a booth at the Middleton Chamber of Commerce 4th of July event. In consideration of accepting this, executors and administrators, waive and release any and all rights and claims I/we may have against the Middleton Chamber of Commerce, the City of Middleton, and all other parties and representatives, successors and assigned for any loss or damage to property or persons. I agree to all fees, rules, and regulations as state in the application and guidelines.
